Mandatory Debt Review Clearance Assessment Forms

Please download, complete, sign and return these documents.

Please ensure that the following documents are also submitted:
1. Copy ID - Not Photo
2. Your Affidavit 

Please note that these documents are going to be used in legal presentations and will be served to all concerned parties and their legal representative and (in extreme circumstances) they might reach the courts. Therefore it is important to send good qualify documents in pdf format (scanned).

In other words return them in the same qualify your received them. Poor quality documents will be rejected.

NCR FORM 29

This is a prescribed NCR dispute form.
This form is compulsory for any dispute regardless of the issue being disputed.
When completing this form ensure you only mention incidents, communication and correspondence with the debt counsellor.
Do not mention or send any correspondence with other parties. The dispute is between yourself and the debt counsellor who placed the debt review flag on your credit records.

Power of Attorney

The power of attorneys is required to authorize access your credit reports from any NCR registered Credit Bureaus and to act on your behalf to request the concerned parties to cooperate in sharing necessary information relating to your debt review situation

NCR THIRD PARTY CONSENT

This is the form that you sign to authorize AACS to log a dispute on your behalf. This form is compulsory regardless the issue being disputed.
